Key Responsibilities:

Patient Support:

Respond to patient inquiries, providing basic medical information, education, and guidance based on established protocols.

Assist patients in understanding treatment plans, medication schedules, and follow-up care.

Appointment Scheduling and Coordination:

Schedule and manage patient appointments, including follow-ups and consultations.

Coordinate with various departments to streamline patient services and appointments.

Telehealth Assistance:

Prepare patients for virtual consultations, ensuring they understand the process and have the necessary technology access.

Support healthcare providers during virtual visits, assisting with documentation and patient communication as needed.

Documentation and Record-Keeping:

Maintain accurate and updated patient records using the Electronic Health Record (EHR) system.

Document patient interactions, symptoms, and care instructions in accordance with healthcare guidelines.

Patient Education and Resources:

Provide educational resources to patients regarding preventative care, wellness, and chronic disease management.

Assist patients in understanding instructions for home-based care and self-monitoring techniques.

Care Coordination:

Coordinate referrals, lab results, and prescription renewals under the supervision of the healthcare team.

Follow up with patients post-appointment to ensure understanding of care instructions and address any further questions.

Administrative Support:

Support the healthcare team with additional administrative tasks as needed, including updating schedules, processing paperwork, and managing patient communications.

Qualifications:

Education: Diploma or Degree in Nursing, General Nursing and Midwifery (GNM), or related field.

Experience: 3 years of experience in a healthcare or telehealth setting, with experience in virtual or remote support preferred.

Technical Skills: Proficiency in using EHR systems, telehealth platforms, and other communication tools.
