1 Conduct initial assessments of patients' health status and needs.

4 Monitor patients' vital signs and other health indicators, reporting any significant changes to the registered nurse or physician.

5 Educate patients and their families about their health conditions, medications, and self-care techniques.

6 Coordinate with other healthcare providers involved in patients' care, including therapists, social workers, and home health aides.

7 Document all patient care activities accurately and timely in accordance with organizational policies and regulatory requirements.

8 Ensure compliance with infection control practices and other safety standards during patient care delivery.

9 Participate in interdisciplinary team meetings to review patients' progress and adjust care plans as needed.

10 Provide emotional support and companionship to patients and their families, fostering a trusting relationship.

11 Maintain confidentiality of patients' health information and adhere to ethical standards of nursing practice.

12 Stay updated on current trends and best practices in home healthcare through continuing education and professional development activities.
