Position Overview:
As a Hospital Pharmacist, you play a crucial role in ensuring the safe and effective use of medications within the hospital setting. Responsibilities:
Medication Dispensing:
Review and process medication orders from healthcare providers.
Dispense and compound medications accurately, considering dosage, form, and route of administration.
Patient Consultation:
Provide medication counseling to patients, ensuring they understand proper usage, potential side effects, and any other relevant information.
Address patient inquiries and concerns regarding medications.
Drug Information:
Stay updated on pharmaceutical products, drug interactions, and new developments in the field.
Provide drug information to healthcare professionals, including physicians and nurses.
Clinical Pharmacy Services:
Collaborate with the healthcare team to participate in patient care rounds and provide input on medication therapy management.
Conduct drug utilization evaluations to ensure safe and cost-effective medication use.
Pharmacy Operations:
Oversee the procurement, storage, and inventory management of pharmaceuticals.
Ensure compliance with regulatory standards and maintain accurate records.
Compounding and IV Admixtures:
Prepare and compound sterile and non-sterile medications, including intravenous (IV) admixtures, in accordance with established protocols.
Quality Assurance:
Monitor and assess the quality and effectiveness of pharmaceutical services.
Participate in quality improvement initiatives within the pharmacy department.
Drug Formulary Management:
Collaborate with the medical staff to manage the hospital's drug formulary.
Evaluate new medications for formulary inclusion.
