Gather pertinent information about the patients medical and family record information in the forms

provided.

Coordinate with the counsellors and patients to assist in organizing appointments while closely working

with the patient caregivers and doctors/hospitals team.

Assist cancer patients/caregivers with accessing all the available resources for cancer patients that are

provided by the doctor/hospital.

Work with the central team to provide counseling support and community program support to patients

based on their need.

Provide emotional support to the patients as needed.

Be an advocate for the patient.
