Patient Care Coordinator - Job Description
Job Responsibilities
Apply knowledge of services and clinical standards to everyday tasks by discussing and developing patient care plans with the care team.
Explain to customers the role of the service provider initiating the care plan.
Ensure complete coordination of information and care requirements with other care providers; resolve issues that could affect smooth care progression.
Maintain and update patient medical records; Coordinate case management process updates with peers and monitor their daily patient care activity reports.
Provide educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements to patient/family members; meeting them periodically (as and when required)
Update patient health records, including admissions and insurance data; Provide medical information to financial services for maximum health facility and patient reimbursements.
Follow facility guidelines, healthcare laws, and regulations
Establish and maintain relationships with service providers
Maintain professionalism and treat patients with respect

Qualification: Any Undergraduate Medical Degree/Hospital Management/Administration Degree
