Job Description

1. Check patient files for incompleteness (registration forms, case sheet, consent forms etc.)

2. Inform concerned department in-case file is incomplete

3. Follow-up with the concerned department/personnel and ensure the details in the files are completed

4. Scrutinize the file thoroughly before stamping it as completed

5. Filing of records in an orderly manner

6. Reports & records matching in ARTis

7. Compliance as per QAI standards

8. Closely working with Clinical team, Nursing and Counsellors and PRE’s

9. Monthly analysis of incomplete and complete files with various parameters in excel sheets
