• Review patient charts and documents and determine the correct codes.

• Ensure proper coding with correct sequence based on the medical records and make sure that all the codes are active for the Date of services.

• Escalate for missing or incomplete reports and unsigned records.

• Determine the codes as per the coding guidelines and ensure compliance with state and federal regulations.

• Check the LCD as per the carrier specifications.

• Knowledge of handling the PQRS measures.

• Follow payer guidelines and identify the reimbursable procedures.

• Address all the coding related queries escalated by charge entry and AR team.

• Regular feedback to the charge entry team and AR team based on the findings.

• Document all the activities in the control log.

• Meet productivity and quality standards as set by the department.

• Assist with month-end closing activities

• Adhere to all company policies and procedures.
