As an ElderAid Care Manager, you will support senior citizens live life fully. Whether it is taking them to the hospital, arranging doctor visits, or running errands, our Care Managers are caring and compassionate people who care for their elders. This job is a field job, and you will have the opportunity to meet and interact with many elders.
Specific job responsibilities include:
Customer-Facing Responsibilities:
- Manage the relationship with the customer
- Track customers’ needs and update/modify Care Plan as needed
- Facilitate/enable the delivery of services to the customer, with support from the Care Executive
- Be the single point of contact for both the elder and their family
- Conduct frequent telephonic check-ins with the customer, and occasional in-person check-ins
- Update the customer’s family regularly and in a timely manner
- Verify and update any documents, reports, etc.
- Conduct quality checks and collect customer feedback
- Generate detailed reports for the customer
- Verify and update customer details as needed
- Up-sell relevant services to the customers
- Coordinate Caretaker Placement
Internal-Facing Responsibilities:
- Manage a team of Care Executives
- Ensure Care Executives are collecting detailed information and updates from their customers
- Ensure Care Executives are delivering timely services while maintaining high standards of quality
- Track, manage, and ensure unit-level profitability
- B2B networking
- Generate and present regular reports to the Management Team
- Plan and organise Wellness Events with support from the Sales and BD Team
- Manage and update the CRM system
